I use a Fritz!Box at the moment.
It has some issues with the DSL Modem (see my review on here) so I use in ATA mode.
The VoIP functionality is quite rich - the best I have found so far:
- 2 FXS lines
- 1 FXO or ISDN line
- Ethernet or USB connection
- 10 SIP accounts
- each SIP account can ring both, either or neither phone.
- Comprehensive dialling rules to switch dialling over any of the 10 SIPs or PSTN. (I set 0800, 999 and 100 over PSTN always)
- Auto area code and IDD preprend
- Call blocking
- Night Service
- PSTN passthrough on internet failure (you get a warning beep)
- Call through (call your box from your mobile, say, and make calls over VoIP)
- caller id, call waiting, 3 way calling, other line call pickup, plus loads of other stuff you get on a full PBX.
- Even a baby monitoring service!
Not really in wide use here, but worth a look - just don't use it as a modem yet.
